Overview

CHIA collects cost reports from Community Health Centers’ (CHC). The reports are used by CHIA and other public entities to monitor costs and to develop health care payment policy. CHCs must submit their data in accordance with 957 CMR 6.00

Reporting Requirements

CHCs are required to submit the following by July 7, 2026.

Community Health Center Cost Report (Excel Template)
Please read the CHC Cost Report Instructions carefully before you begin working on the cost report. Agencies must file the cost report based on the provider’s fiscal year end. The reporting period shall be consistent with financial statements. The cost report needs to be completed on the basis of the total program costs.

Submit completed cost reports through CHIA’s online submission portal, CHIA Submissions. If your agency has not registered with CHIA or if you cannot find your provider name in the CHC Cost Report template, you must complete a Data Reporter User Agreement.

Financial Statements
Agencies must submit copies of financial statements and other external documentation supporting the accuracy of the data reported on the cost report. See the CHC Cost Report Instructions for a list of acceptable documentation 

Email financial statement documents data@chiamass.gov. Label financial statements files with the CHC name and document type. For example:
AgencyName_CHCFS_FY25.pdf

Registration Instructions

New users must register to submit data via CHIA Submissions.

  1. Complete and sign the CHIA Data Reporter User Agreement after reviewing the DRUA instructions. Note that each individual user for an organization must complete the form.
  2. Submit the completed Data Reporter User Agreement to DL-Data-Submitter-HelpDesk@chiamass.gov.
  3. You will be notified when your account is ready to use.
